PCNL (Percutaneous Nephrolithotomy)

Percutaneous Nephrolithotomy (PCNL) is a minimally invasive surgical procedure used to remove large or complex kidney stones. It involves making a small puncture (approximately 1 cm) through the skin to access the kidney directly.

This procedure is especially effective for stones larger than 2 cm or those located in the renal pelvis. PCNL is typically performed under general or spinal anesthesia.

Advantages:

  • Highly effective for large kidney stones
  • Minimally invasive with a small incision
  • Faster recovery compared to open surgery
  • High stone clearance rate

Risks:

  • Bleeding
  • Infection
  • Injury to surrounding organs (rare)

URSL (Ureteroscopy and Laser Lithotripsy)

URSL is an advanced endoscopic procedure used to treat stones located in the ureter. A thin ureteroscope is passed through the urethra into the bladder and ureter without any external incision.

A laser fiber or lithoclast is used to break the stone into tiny fragments, which either pass naturally or are removed using a basket device.

Key Features:

  • No external cuts or stitches
  • Procedure duration: approximately 30 minutes
  • Usually performed as a day-care procedure
  • High success rate, especially for lower ureter stones

In some cases, a Double-J (DJ) stent may be placed temporarily to ensure proper urine flow and healing.

Advantages over ESWL:

  • Effective for hard stones
  • Immediate stone fragmentation and removal
  • Dilates ureter to facilitate stone passage

Limitations:

  • Stones in the upper ureter may move back to the kidney
  • May require additional treatment like ESWL in some cases

TURP (Transurethral Resection of the Prostate)

TURP is a surgical procedure used to treat urinary problems caused by an enlarged prostate. It involves inserting a specialized instrument through the urethra to remove the portion of the prostate blocking urine flow.

Procedure Details:

  • Performed under general or spinal anesthesia
  • Requires a hospital stay of 1–2 days
  • A catheter is placed temporarily to drain urine and blood clots

Recovery:

  • Catheter is removed once urine clears
  • Avoid strenuous activities for 4–6 weeks
  • Temporary urinary symptoms may persist but improve gradually

TURBT (Transurethral Resection of Bladder Tumor)

TURBT is a minimally invasive endoscopic procedure used to diagnose and treat bladder tumors. The entire procedure is performed through the urethra using a specialized scope, without any external incision.

Procedure Overview:

  • Performed under anesthesia in a hospital setting
  • Tumor is removed using a cutting instrument
  • Area is cauterized to control bleeding and prevent recurrence

Purpose:

  • Diagnosis and staging of bladder tumors
  • Treatment of superficial bladder cancers
  • Helps determine need for further therapy

Post-Procedure Symptoms:

  • Mild burning or discomfort during urination
  • Temporary changes in urine flow
  • Blood in urine for 1–2 days
  • Occasional minor bleeding after 1–2 weeks due to healing
